Payment Request Form

A completed form and detail summary justifying the charges should be submitted to KDOT Design Contracts section, 12^th Floor, Eisenhower State Office Building, Topeka, Kansas 66603-3745 for each consultant payment request. This information can also be forwarded via e-mail and fax which are noted on the request form.
