KANSAS TRUCKING CONNECTION (KTC)
SUPERLOAD POST-TRIP REPORT INFORMATION
Certified escort drivers reporting problems and special conditions is the best way for the KTC to obtain information about the movement of superloads. Therefore, this form is an essential part of the Analysis of Superloads.

FORM REQUIREMENTS
Fill out the form completely and accurately
The information supplied on this form will be used to determine better routes in the future.
The form shall be completed by only one of the escort drivers and signed by two escort drivers.
A second escort shall also sign the report to indicate he/she has read the report and the information included in the report is accurate.
The name of each escort driver shall be included in the report along with their certification number.
When the move has been completed and this report has been filled out, one of the escorts shall mail this report to the KTC. This report shall be received by the KTC within 5 days following the end of the move.

ESCORT RESPONSIBILITIES
Do not proceed with a superload move until there are a minimum of two certified escorts present to escort the load. If an escort must leave during the move the load shall be stopped until a second escort can be found.
Make sure the escorts and the driver of the superload has the proper credentials (permit and bridge report, etc.) in-hand before moving
Make sure that the dimensions of the load correspond to what is shown on the permit/Superload bridge review memorandum
Make sure the load is secured so that the load will not fall off or become dislodged from the transporting vehicle
Make sure that all escort drivers and the driver of the superload know the proper route, the location of important structures, and those locations where special attention needs to be concentrated order to proceed safely through the obstruction.
When crossing bridges, make sure there are no other vehicles crossing the bridge at the same time as the superload and reduce the speed of the transporting vehicle to to correspond with the speed shown on the “KDOT Superload Review.”
Make sure that all local enforcement officials are present if and when they are needed to control traffic.
Ensure that all appropriate electrical and railroad officials have been contacted before moving
Contact KDOT to clarify all conflicting issues before moving the superload. Call Bridge design at 785-296-4434 or the KTC at 785-271-3145 (option 2).

TYPE OF ACTIVITIES TO REPORT

Roadway restrictions that were not noted on the permit
Hazards along the way that may preclude other moves
Problems with other vehicles along the way
Cause for delays
Damage to any vehicle, property or roadway structures caused by the load coming into contact with a bridge or other object(s) along the highway
Inaccuracies surrounding the permit - if this happens, a call to the KTC should be made immediately to correct the problem before proceeding with the move
How each reported incident was corrected
Truck drivers not transporting the load in compliance with the “KDOT Superload Review” or the OSOW permit.
Injuries to anyone during the move
Conditions surrounding a warning or summons issued to the driver(s) involved with the move
Anything else the escort driver(s) feel need to be relayed to the KTC

The purpose of reporting escort activity to the KTC after each move is to continually improve the superload process, not to blame anyone. It is better to correct problems when they are small and easy to solve. If further intervention is needed, inform the KTC immediately. Support will be provided as soon as possible.
